Cusco part 2

So my first stop from Lima was Cusco, I was here with my Gorgas course but got sick, so finally did Machu Picchu, rainbow mountain, bought more things at the market! There are tons of tourists here and lots of dirty hippies. Machu Picchu was great cause the day was perfect, after went to this spa Agua calientes which is apparently hot springs, but judging by the amount of children there, would assume there is a high amount of urine present. Rainbow mountain, color formed by unique sediments, the top is 5000 meters high, super tiring walking even a few feet. Our guide gave us some liquid stuff to inhaled, i'm sure it was some cocaine derivative! wahoo Also, as mentioned above regarding hippies here, also brings lots of vegan food, some of the best Vegan food is in Cusco, its what I search for now. Finishing Spanish classes... for now

I've been two weeks in Lima now doing intensive Spanish courses and took my written and oral exam and have graduated! to... pre-intermediate :P I plan to travel and continue my studies. I finally ended up going surfing which pretty fun despite my instructor being quite confusing with lessons. Ended up on a food tour in Barranco, tried lots of traditional Peruvian foods like Cow's heart and this other meat dish surrounded by Potatoes. Things got a little weird when he told us when he did meth and licked a hookers but one time.... but other than that it was great! I'm off to Cusco Today.

9 weeks later

Had my bag stolen at the start of the course with computer which really took the winds out. But now the course is finished, was a lot of fun and met some nice people. Now I ´m in an intensive Spanish course trying to learn as much as possible before my big travels. I am also trying to do some activities I didn´t get a chance to do during the course, went surfing for the first time yesterday! Gonna do paragliding today!

Day 2, TB TB TB

On day two we have further talks about Tuberculosis, Dr. Evans gave a fantastic talk on tuberculosis, he spent a great time helping us understand the issues of transmission as well as who is considered high risk. Patients malnourished being a large risk factor for active TB and even showed some data that if you look at population attributable fraction, malnourishment is a greater risk of disease over HIV! He had mentioned that patients that have been treated for latent TB but re-exposed, may not be protected, hence the idea of treating people in highly endemic areas might not be cost effective. We had another talk on presentations of TB by Dr.Gotuzzo. He was also great. Reviewed positive skin test (which varies from Canada, US, WHO), but 5 mm positive in HIV, recent contact, fibrotic change son CXR, organ transplant on immunosuppressed patients. Cases today 47 yo male from a coast city but lives in Lima, has previously travelled to the Jungle last year. He is known HIV, issues ...

Day 1 officially: Malaria, TB and cool cases

Day 1 was awesome. I met more of my classmates and everyone has a pretty interesting story. We have small groups for case rounds and a separate small groups for labs to meet more people.  Really great lecture on Malaria, that went over finer details of pathophys. I really liked how he talked about how the definitive host is the mosquito, and how gametes are picked up, goes to abdo of mosquito, come together for ookinete which penetrates intestinal cell, comes out as an oocyst which then makes 8000 motile sporozoites which migrate to salivary glands.  Further reminder of PfEMP-1 of P. falciparum to ICAM causing cyto-adherence and sequestration. Stages of the parasite in humans are: ring trophozoite, ameboid trophozoite (P.vivax), and band (P. malariae, P. Knowlesi), immature then mature schizont, and then gametocytes. Maurer's clefts are found in  P. Falciparum. P. falciparum develop in the bone marrow. ...
